1. Agreement to Terms
By using the EZPHLEB website, booking an appointment, submitting a contact form, requesting a partner quote, calling, texting, emailing, or otherwise contacting EZPHLEB, you agree to these Terms of Service.
2. Services Provided
EZPHLEB LLC provides mobile phlebotomy and specimen collection support in Tampa Bay, Florida. Services may include routine mobile blood draws, specialty kit collection, specimen pickup and delivery, same-day or after-hours draw options, corporate wellness or group draw support, and service at a home, office, assisted living facility, care facility, clinic, or business location.
3. Service Limitations
- EZPHLEB is not a laboratory.
- EZPHLEB does not diagnose medical conditions, treat patients, or interpret lab results.
- EZPHLEB does not replace a doctor, laboratory, or healthcare provider.
- EZPHLEB follows instructions provided by the doctor, lab, or specialty kit when available.
- EZPHLEB does not guarantee that a lab will accept, process, or result a sample.
- EZPHLEB does not guarantee laboratory turnaround time, shipping timelines, or lab results.
4. Patient and Customer Responsibilities
Customers should provide accurate contact, address, and appointment information. Before the visit, customers should have ready:
- Lab order or requisition if required.
- Photo ID.
- Insurance card if the lab requires it.
- Specialty kit and instructions if applicable.
- Fasting or timing instructions if required.
- Access instructions, gate code, parking notes, or facility contact details if applicable.
5. Specialty Kits
For specialty kit collection, the patient or customer is responsible for having the kit, forms, labels, tubes, shipping materials, and instructions ready unless otherwise arranged. Some kits may require timing, fasting, centrifugation, refrigeration, freezing, or specific shipping instructions.
EZPHLEB follows provided kit or lab instructions when available, but does not guarantee lab acceptance, processing, turnaround time, or results.
6. Pricing
Current EZPHLEB mobile collection service pricing:
- Routine Blood Draw: $99 flat fee / visit
- Specialty Kit Collection: $125 flat fee / visit
- Same-Day & After-Hours Draw: $150 flat fee / visit
- Specimen Pickup & Delivery: $50 flat fee
- Corporate Wellness & Group Draw: Custom pricing
- Additional person during same visit: +$75
- Additional specialty kit during same visit: +$75
Prices may not include laboratory testing fees, provider fees, shipping fees, insurance charges, or charges billed directly by a lab, doctor, specialty kit company, carrier, or other third party.
7. Appointments and Scheduling
Appointments may be scheduled online, by phone, or through direct communication with EZPHLEB. Availability may vary by date, location, service type, and operational needs. Same-day and after-hours appointments are subject to availability. Customers should provide accurate information so the visit can be coordinated properly.
8. Cancellations and Rescheduling
Appointments may need to be rescheduled due to access issues, missing orders, unavailable kits, incomplete information, weather, safety concerns, operational constraints, or other circumstances that prevent proper service. EZPHLEB may contact the customer to resolve the issue or identify another appointment time.
9. Insurance
EZPHLEB may collect insurance information if needed by a laboratory or third party. EZPHLEB's mobile collection service fee may be separate from laboratory testing, provider billing, insurance billing, or other third-party charges. Insurance coverage is not guaranteed by EZPHLEB.
10. Specimen Pickup and Delivery
For specimen pickup and delivery, the specimen should be properly sealed, labeled, and ready for pickup. A lab requisition or related documentation may be required. EZPHLEB may decline pickup or request rescheduling if the specimen is not properly prepared, not safe to transport, missing required documentation, or inconsistent with the instructions provided.
11. Safety and Access
Safe access to the appointment location is required. Customers should provide parking instructions, gate codes, facility details, and any other access notes needed for the visit. Pets should be secured if needed. EZPHLEB may reschedule if conditions are unsafe or if access to the location cannot be completed in a reasonable manner.
12. No Emergency Services
EZPHLEB is not an emergency medical service. For medical emergencies, call 911 or seek urgent medical care immediately.
13. Website Use
Users should not misuse the website, submit false or misleading information, attempt unauthorized access, interfere with website operation, or use website forms or booking tools for unlawful, fraudulent, or abusive purposes.
14. Third-Party Services
EZPHLEB may use third-party services such as Acuity Scheduling, Formspree, Google Analytics / GA4, Google Tag Manager, Google Ads if used, WhatsApp, email providers, phone providers, laboratories, kit companies, or shipping carriers. Third-party platforms and providers may have their own terms, privacy practices, timelines, processing requirements, and operational limitations.
15. Limitation of Liability
To the fullest extent allowed by law, EZPHLEB is not responsible for delays, rejected samples, missing results, billing issues, shipping issues, processing issues, incorrect information, missing orders, incomplete kit materials, kit requirements, carrier delays, third-party lab decisions, website platform issues, or other circumstances caused by third-party labs, shipping carriers, incorrect customer information, missing documentation, external systems, or events outside EZPHLEB's reasonable control.
16. Changes to Terms
EZPHLEB may update these Terms of Service from time to time. Updates will be posted on this page with a revised last updated date.
